Mẫu IMM5645E.pdf, gồm 2 trang là thông tin về gia đình, anh chị em ruột,…
Mẫu Studypermitquestionaire, gồm 2 trang là thông tin về tài chính bố mẹ, người thân Canada,…
Sau khi hoàn tất bộ form này các bạn sẽ xếp trang mã vạch lên đầu tiên, tiếp theo là IMM 1294E, IMM5645E và Study permit questionnaire và gộp các form này vào bộ hồ sơ xin visa du học.
