Canadian Visa application forms

To ensure a complete and accurate study visa application for Canada, applicants must fill out several mandatory forms. Each form provides essential information required by immigration authorities and must be fully completed, dated, and signed. The following section outlines the required forms and the correct order for assembling your application package.
- Form IMM1294E – 4 pages (Note: This form must be completed online and “Validated” to generate the barcode pages, then printed directly). This form includes information about the student, the school, tuition fees, and program details.
- Form IMM5645E (Family Information) – 2 pages: This form contains information about the applicant’s family members, including parents and siblings.
- Study Permit Questionnaire – 2 pages: This form provides information on the parents’ financial status, sponsors, and any relatives in Canada.
After completing all forms, arrange them in the following order:
- Barcode pages (from IMM1294E)
- Form IMM1294E
- Form IMM5645E
- Study Permit Questionnaire
Then combine these forms into the complete study visa application package.
