Bảo trợ- giám hộ học sinh dưới 18 tuổi

Với học sinh dưới 18 tuổi du học tại Canada, việc bảo trợ học sinh là bắt buộc. Tờ khai ủy quyền từ cha mẹ học sinh/ nhận ủy quyền của đơn vị hoặc cá nhân bảo trợ học sinh cần có công chứng hoặc có chứng nhận chữ ký của chính quyền địa phương do cha me hoặc người giám hộ hợp pháp ký chấp nhận trao quyền giám hộ cho người bảo lãnh tại Canada và nêu rõ họ tên người giám hộ ở Canada và người này thực hiện nhiệm vụ thay thế cha mẹ đứa trẻ trong các trường hợp khẩn cấp như cấp cứu hoặc cần sự can thiệp đột xuất. Các tờ khai này có sẵn tại đây và gồm 2 mẫu đơn:
- Đơn dành cho người giám hộ ở Canada, người này phải trên 21t, quốc tịch Canada, sống gần trường của học sinh, người giám hộ ở Canada phải hoàn tất form này có công chứng và gởi về VN cho học sinh. Trường hợp học sinh không có người thân giám hộ ở Canada thì có thể nhờ trường giám hộ.
- Đơn dành cho cha mẹ du học sinh- đồng ý đề trường/ cá nhân hợp pháp giám hộ. Cha mẹ học sinh chỉ cần dịch song ngữ Việt -Anh và điền hết tất cả thông tin vào form này, sau đó ra phường ký tên trước mặt người công chứng
